  Please complete the baptism registration form below and a member of our Faith Formation Office will be in touch with you. This form is for infant and child baptism only. If you are an adult who wishes to be baptized, please enroll in our RCIA program by filling out an inquiry form. If you have a child age 11 or older who needs baptism, please contact our Youth Minister Ed Snyder ([email protected] | 919-570-0070 Ext. 135).
  
  Checklist to register for Baptism:
  Fill out the registration form below!
  Register for and attend a Baptism Preparation Class. This is required for the parent(s) and godparent(s) of the child to be baptized.
  Have the godparent(s) complete and submit to us a godparent suitability form. This needs to be signed by the parish attended by the godparent prior to submission.
  E-mail us (see contact info below) a copy of your child's birth certificate.
  Once we receive these documentary requirements, we can proceed with scheduling a baptism date.
